This World Immunization Week — which takes place every year at the end of April — WHO is launching “The Big Catch-Up.” This global push complements the work already happening in many countries to ramp up vaccination efforts. In essence, it aims to do exactly what it sounds like: catch kids up on the vaccines they missed during the pandemic. The Big Catch-Up also has a particular focus on reaching those kids who have never received a single dose of any vaccine. Because of the pandemic, there are 5 million more kids who are completely unprotected from dangerous, deadly diseases. To be most effective, The Big Catch-Up is focusing on those countries that saw the steepest declines during the past few years.

On top of this short-term goal, The Big Catch-Up is designed to help strengthen local health systems so that immunization is integrated into primary health care so we don’t see disruptions on the scale of what we saw with COVID-19 ever again. Pakistan launches novel initiative to reach children

What is that Polio Picture?  Pakistan’s polio program has launched a novel initiative to reach children of nomadic families with polio and essential vaccines so that they are no longer left behind.

Novel Type 2 Oral Polio Vaccine – Approximately 595 million doses of nOPV2 have been administered across 28 countries under its WHO Emergency Use Listing (EUL) to date. An additional 14 countries have met the requirements for nOPV2 use in the event of an outbreak. Take a look at the nOPV2 webpage or more information on the vaccine.

Novel Oral Polio Virus Type 2 Vaccine

What is that Polio Picture? Novel Oral Polio Virus Type 2 Vaccine

In 2011, a major endeavor was launched to develop a more genetically stable version of the monovalent oral Polio vaccine type 2 (mOPV2) to more sustainably stop outbreaks of type 2 variant poliovirus.

Fast forward to today, and close to 600 million doses of the novel oral polio vaccine type 2 (nOPV2) have been used across 28 countries in outbreak response. More than two thirds of all countries that have used nOPV2 for outbreak response did not report continued transmission following two vaccination campaigns. In some areas with persistent transmission, we’ve seen interruption [of transmission] after a third round. There are, however, a few geographic settings where the virus persists despite multiple campaigns, and these are the areas where we need to focus more on reaching missed children with the vaccine, to stamp out the remaining reservoirs of transmission.

Overall field effectiveness of nOPV2 in stopping paralytic outbreaks seems to be comparable with Sabin mOPV2, which is in line with clinical study findings. Most importantly, with lower risk of seeding new outbreaks compared to the Sabin mOPV2 experience as noted by field data so far, nOPV2 continues to demonstrate that it is an excellent tool to more sustainably stop outbreaks. Polio is a crippling and potentially fatal infectious disease. There is no cure, but there are safe and effective vaccines. The strategy to eradicate polio is therefore based on preventing infection by immunizing every child until transmission stops and the world is polio-free.

Polio program provides assistance to Measles effort Afghanistan

What is that Polio Picture? The Polio program provides vital assistance to Measles effort in Afghanistan. 

With more than twenty years’ experience on the ground in Afghanistan, WHO’s polio eradication programme continues to leverage its extensive operational capacity to deliver better health outcomes for all Afghans, including providing vital support to the recent nationwide measles vaccination campaign.

Measles outbreaks were reported across Afghanistan throughout 2022, with more than 5,000 cases and an estimated 300 deaths reported by November. Complications from the measles virus include severe diarrhea and dehydration, pneumonia, ear and eye complications, encephalitis or swelling of the brain, permanent disability and death. Most cases are children under the age of 5 years. Like Polio, there is no treatment for measles, the only reliable protection is vaccination.

While a series of sub national measles vaccination campaigns took place in 2022 reaching approximately three million children in 141 districts, the nationwide campaign from November 26 to December 5 represented the first national measles drive since the political transition in August 2021. The campaign covered 329 districts in all 34 provinces, vaccinating 5.36 million children aged between from 9 to 59 months against measles. 6.1 million children between 0 to 59 months received oral polio vaccine.

WHO’s polio eradication program has significant reach in Afghanistan, with a presence in every district in the country. The polio program leveraged this presence to recruit vaccinators, organize vaccination sites, and train campaign staff. With longstanding relationships with local authorities, the polio program assisted in the selection of local schools, clinics, or mosques to serve as vaccinations sites. The program’s established relationships with health institutions and communities enabled polio staff to recruit local health workers and other staff to fill the roles of measles vaccinators and provide training. Sharing their experience of implementing polio vaccination campaigns helped measles vaccinators prepare and plan for the task ahead.

The detection of measles cases and collection of data by WHO’s extensive polio surveillance network also played a crucial role in providing evidence-based planning for the campaign. WHO’s polio program also provided logistical support, transporting measles and polio vaccines, ensuring the cold chain was maintained and vaccines were delivered to every district. Polio staff played additional roles in campaign monitoring and supervision.

“Measles is a highly contagious disease. WHO Afghanistan is very proud of its work immunizing and protecting children against both measles and polio in this campaign,” said Dr Luo Dapeng, WHO Representative in Afghanistan. “I am very grateful to all health workers, partners and donors who made this possible.”
