Afghanistan – Wild poliovirus transmission in Afghanistan is currently at its lowest level in history. Fifty-six children were paralyzed by wild polio in 2020. In 2021, the number fell to four. This year to date, only one child has been paralyzed, giving the country an extraordinary opportunity to end polio.

Polio Returns to the United States! – You may have recently read a headline something like this from your local or national news source.  And you may have wondered – is real and if so, what’s being done about it?  After all, Global Polio Eradication is still Rotary’s # 1 Priority – isn’t it?  First, let me thank all of you for the amazing work you have already done to bring us that much closer to our goal of a Polio-Free world.  We have already saved over 20,000,000 children from being paralyzed by Polio!  We have reduced the yearly number of the Polio paralyzed by over 99.9% since 1988 and Rotarians have donated over $2 Billion to this effort.  But we have not yet reached our goal. 

Yes, this case is very real and serious.  The U.S. was declared polio free in 1979 and a case had not originated in the country since then, but travelers have occasionally brought the virus into the U.S. The Global Polio Eradication Initiative (GPEI) was informed of a case of paralytic Polio in an unvaccinated individual in Rockland County, NY in July, 2022 and since that time, Poliovirus has been detected in the sewage water in Rockland County and in other areas in and around New York City.  The CDC has confirmed that the Poliovirus causing the paralysis in the individual and in the environmental samples is Type 2 Vaccine Derived Polio Virus – not the Wild Polio Virus.  That means the source of the Poliovirus had to be the live virus, Sabin-like Oral Polio Vaccine given in a country outside of the US, as the injected Polio vaccine given in the US is a killed virus vaccine which is not capable of mutating to a live-infectious form.  The Vaccine Derived Polio Virus can spread and cause paralysis and death  just like the Wild Polio Types.  Additional unvaccinated, infectious individuals could be carrying the virus as most Polio positive cases show mild or no symptoms. 

What is being done about it?  Investigations are underway to determine the extent of the spread of the virus and the potential risk associated with the environmental samples.  Residents in the areas near the Polio positive samples (and everywhere else!) are being advised to make sure that they and their children are vaccinated for Polio   And finally, environmental surveillance for the Poliovirus is being strengthened in order to rapidly detect any new virus importation. 

What can you as a Rotarian do?  This is a wake-up call to the unvaccinated.  Encourage your family and your friends to be vaccinated for Polio.  The individual in New York would not be paralyzed if they had been vaccinated.  Childhood Polio vaccinations last a lifetime.  You can also remind your friends and associates of the great work Rotary has done to get us to this point of nearly eradicating this terrible disease and the work left to reach our goal (with only two remaining Polio endemic countries – Afghanistan and Pakistan).  And finally, responding to outbreaks is costly so please DONATE to The Rotary Foundation’s PolioPlus Fund.  You can do so at Rotary.org/myrotary or Rotary.org/donate.   With the continuing Gates Foundation $2 to $1 match on all Rotary related Polio eradication donations, this time of year, as we approach World Polio Day, is a great time to encourage your club members and friends to donate to Rotary’s # 1 priority – Global Polio Eradication!

What is that Polio Picture? Polio Surveillance Has Never Been More Important – In Yemen, Dr Mutahar Ahmed leads the country’s acute flaccid paralysis (AFP) surveillance efforts, which are vital to tracking Poliovirus transmission. Despite long-running conflict, a critical humanitarian situation and a cVDPV2 outbreak, wide-reaching AFP surveillance in Yemen tells a promising story where health workers are able to detect polio spread and inform response tactics. Along with strong surveillance systems, vaccination campaigns, like continued nationwide campaigns in Afghanistan, are crucial in order to stop all forms of polio everywhere.

WHO thanks the Trustees of the Rotary Foundation approved PolioPlus grants totaling almost US$ 22.5 million at their June 2022 meeting, in support of technical assistance to the African Region, Pakistan operations, global outbreak response, and research projects in the UK and Nigeria.

This is not the first polio outbreak in New York

What is that Polio Picture? This is not the first Polio outbreak in New York.  The 1916 New York City polio epidemic was an infectious diseaseepidemic of polio ultimately infecting several thousand people, and killing over two thousand, in New York City, primarily in the borough of Brooklyn. The epidemic was officially announced in June 1916, and a special field force was assembled under the authority of Dr. Simon R. Blatteis of the New York City Health Department’s Bureau of Preventable Diseases, with broad authority to quarantine those infected with polio and institute hygiene measures thought to slow the transmission of the disease. Polio was a poorly understood disease in this era, and no polio vaccine existed at this time. Official efforts to stem its spread consisted primarily of quarantines, the closure of public places, and the use of chemical disinfectants to cleanse areas where the disease had been present. Special polio clinics were established at various locations in the city for the treatment and quarantine of the patients.

United States Polio Case Update

The Global Polio Eradication Initiative (GPEI) has been informed of a case of paralytic polio in an unvaccinated individual in Rockland County, New York, United States.

Following the detection, the Global Polio Laboratory Network has confirmed that the VDPV2 isolated from the case is genetically linked to two Sabin-like type 2 isolates, collected from environmental samples in early June in both New York and greater Jerusalem, Israel. Further investigations – both genetic and epidemiological – are ongoing to determine possible spread of the virus and potential risk associated with these isolates. The young adult developed paralysis and health experts have confirmed the person is no longer contagious.  The young adult was not vaccinated and did not report recent travel outside the U.S.

 Any form of poliovirus anywhere is a threat to children everywhere!

On 12 June, sports stars and celebrities met on the pitch for SoccerAid, an annual football match to support UNICEF’s efforts protecting the health of children worldwide. This year’s event focused on polio, raising a record-setting £15 673 728 in part to aid the work of the Global Polio Eradication Initiative (GPEI) building a polio-free world. The match made an important call to action – to end all forms of polio for good – which was further underscored when poliovirus was detected in sewage samples in London. While no children have been paralysed by poliovirus in the United Kingdom since 1983, this detection is yet another firm reminder that as long as polio exists anywhere, it remains a threat to children everywhere.
 
Yemen –  Almost two-thirds of the areas reporting the bulk of cVDPV2 cases have not implemented any type 2-containing polio vaccination campaign, underscoring the risk both to local children and to neighboring countries.

What is that Polio Picture?  Some people go to extreme heights to Eradicate Polio – Like the amazing Kurt Matzler who on Saturday completed a solo Race Across America bicycle race (from Oceanside,CA to Annapolis, MD) – a total of 3,017.8 miles which included 74,064′ of elevation gain – in just 11 days, 6 hours, and 15 minutes.  Why, you might ask?  Kurt with his great Support Team Rotary RAAM Polio, have have made thousands more people aware of Rotary’s # 1 Priority – Global Polio Eradication – and have raised over $4 million for the END POLIO NOW cause.  Congratulations to Kurt and to our whole Team!  Watch the video below to see Kurt’s Ted Talk about what it takes to complete an endurance race like the RAAM.
