Pakistan struggles from effects of devastating floods

What is that Polio Picture? As Pakistan continues to struggle from the effects of the devastating floods affecting parts of the country, polio staff on the ground continue to assist emergency relief efforts.

In flood-affected districts, the polio effort is supporting establishment of critical health camps, to provide basic clinical services, particularly ensuring treatment of water-borne and vector-borne diseases, and distributing water purification tablets.  All routine immunization antigens are also provided to target children and pregnant women.  Staff are actively conducting surveillance for communicable diseases, identifying nutrition needs of displaced populations, and collecting and analyzing life-saving data to help target response strategies. 

At the same time, the Polio program is adapting its operations, to ensure polio eradication efforts can continue unabated, even amid the tragedy.  The program is at a critical juncture – intensive response is ongoing to stop this year’s outbreak in southern Khyber Pakhtunkhwa.  Virus linked to this outbreak was this month detected in an environmental sample from Karachi, in the south of the country.  At the same time, the high transmission season for polio transmission is now starting and this transmission risks being particularly intense given the floods.

But despite these challenges, polio staff are working double-time:  adapting polio approaches, while supporting life-saving flood relief efforts.

“I have been fortunate enough to be present when a number of countries successfully eradicated polio,” commented Dr Hamid Jafari, Director for Polio Eradication at the World Health Organization’s Regional Office for the Eastern Mediterranean.  “Rarely have I seen such commitment and dedication than I have seen in Pakistan – from national political leaders, to health workers, right to the mother and father on the ground.  To all who are involved, all I can say is:  Thank you!  You are making a huge difference to people’s lives, which goes far beyond the effort to eradicate polio.”

Immunizations ongoing in Malawi

No new Wild Polio case was reported this week.

What is that Polio Picture? Immunizations ongoing in Malawi.

Malawi – As a result of ongoing disease surveillance, the Global Polio Laboratory Network (GPLN) has confirmed the presence of type 1 wild poliovirus (WPV1) in a child suffering from paralysis in Tsabango, Lilongwe, Malawi. Analysis shows that the virus is genetically linked to WPV1 was detected in Pakistan’s Sindh province in October 2019.

The three-year-old girl in Malawi experienced the onset of paralysis on 19 November 2021, and stool specimens were collected for testing on 26 and 27 November. Sequencing of the virus conducted in February by the National Institute for Communicable Diseases in South Africa and the U.S. Centers for Disease Control and Prevention confirmed this case as WPV1.

Detection of WPV1 outside the world’s two remaining endemic countries, Pakistan and Afghanistan, is a serious concern and underscores the importance of prioritizing polio immunization activities. Until polio is fully eradicated, all countries remain at risk of importation and must maintain high vaccination coverage to protect all children from polio.

The GPEI is supporting health authorities in Malawi to conduct a thorough assessment of the situation and begin urgent immunization activities in the sub-region to mitigate any risk of spread. Surveillance measures are also being expanded in Malawi and neighboring countries to detect any other potential undetected transmission.

As an imported case from Pakistan, this detection does not affect the WHO African Region’s wild poliovirus-free certification status officially marked in August 2020. Malawi last recorded a case of wild poliovirus in 1992. The polio eradication program has seen importations from endemic countries to regions that have been certified wild poliovirus-free in the past and have moved quickly to successfully stop transmission of the virus in these areas.

 

 Afghanistan

After the safe and successful rollout of nationwide polio campaigns since November 2021, we have received the tragic news that 8 health workers at the forefront have been killed this morning in a series of shootings in Takhar and Kunduz in northeast Afghanistan.  The vaccination campaign has been suspended in both provinces. 

A statement has been issued by the Regional Director of WHO’s Eastern Mediterranean Region, Dr. Ahmed Al-Mandhari, as well as by the United Nations in the country, condemning these attacks, expressing condolences to the families, and underscoring that the provision of health and the safety of health workers at the forefront are paramount, and must be kept neutral to any geo-political situation anywhere.

Our thoughts and prayers are both with the families and our teams on the ground at this time.

The 2022-26 Polio Eradication Strategy underscores the urgency of 2021-2026 Polio eradication efforts and offers a comprehensive set of actions that will position the GPEI to achieve a Polio-free world!

What is that Polio Picture? This picture shows Ellyn W. Ogden, MPH.

On Ellyn Ogden’s desk sits a decades old coffee mug. It has a special significance for the woman who has led polio eradication for the US Agency for International Development (USAID) since 1997. On the mug, are ten women, the Ladies in Polio, who at the time were the only women participating in GPEI meetings, and only 2 of them were in leadership positions.  Ellyn takes her status as a leader in polio eradication seriously and by her actions demonstrates commitment, determination and the need to engage women at all levels of GPEI.  As an epidemiologist, public health practitioner and trained ’devil’s advocate’ Ellyn oversees what is cumulatively a $1.3 Billion investment, spanning 30 countries and covering all aspects of polio eradication from surveillance to communication to containment to NGO and civil society engagement.  She is the only female member of the European Certification Commission for polio eradication.
