This week, history was made in Tajikistan as the country became the first to close an outbreak of type 2 variant poliovirus (cVDPV2) using the novel oral polio vaccine (nOPV2). Vaccination campaigns began in February 2021 using nOPV2, a new tool that can help to more sustainably stop outbreaks, and by August, all traces of the virus had been wiped out in Tajikistan. Thanks to the concerted effort of GPEI partners, national authorities and health workers in communities across the country, we now have a promising look into how nOPV2 can help us build a healthier future.

Last week, the Global Polio Eradication Initiative (GPEI) announced that it is seeking new commitments (to total $4.8 Billion) to fund its 2022-2026 Strategy at a virtual event to launch its investment case.  The strategy, if fully funded, will see the vaccination of 370 million children annually for the next five years and the continuation of global surveillance activities for polio and other diseases in 50 countries.

The investment case outlines new modeling that shows achieving eradication could save an estimated US $33.1 billion this century, compared to the price of controlling polio outbreaks.

New Reported Case in Pakistan

What is that Polio Picture? This picture shows Pakistan – After nearly 15 months without any reported cases of wild polio in Pakistan, a 15-month-old boy in North Waziristan was paralyzed by the wild poliovirus.

Pakistan’s Polio Emergency Operations Centers have deployed teams to conduct a full investigation of the recent case, while emergency immunization campaigns are underway to prevent further spread of the wild poliovirus to other parts of Pakistan and across the Afghan border.

While this news is disappointing, it is not entirely unexpected. Despite record-low case counts, throughout 2021, polio surveillance in Pakistan had detected positive environmental samples indicating that the virus continues to circulate in parts of the country. As the Global Polio Eradication Initiative (GPEI) responds urgently to the new case, Rotary members in Pakistan will work to raise awareness about the importance of polio immunization and advocate with the government to prioritize eradication.

The case in Pakistan underscores the fact that our work is not over, and any complacency can hinder the polio program’s progress. It is critical that Rotary members around the world intensify their efforts to raise funds for PolioPlus, so that we can provide the funding necessary to respond to any new cases of polio in polio-endemic and outbreak countries. At the same time, Rotary members can be proud of the work they have done to bring wild polio case counts to a historic low, and can continue to leverage this momentum to make polio eradication a reality.

During World Immunization Week, 24-30 April, let’s advocate for polio eradication and the importance of vaccines to the health and well-being of our communities. 

Vaccines are one of the greatest advances of modern medicine. They’re safe, effective, and save millions of lives each year. Our progress in the fight to end polio is proof that vaccines work.
The Global Polio Eradication Initiative estimates that the polio vaccine has saved more than 20 million children around the world from paralysis. We need to continue vaccinating children against polio, because as long as the poliovirus circulates anywhere, it is a threat everywhere.

Here’s how you can support Rotary’s efforts during World Immunization Week:

  • Use the World Immunization Week Toolkit for graphics and sample posts to share on social media with the hashtags #EndPolio and #VaccinesWork.
  • Donate to PolioPlus to help Rotary and our partners deliver polio vaccines to vulnerable children. Or start an online fundraiser using Raise for Rotary and share it with your friends, colleagues, and family members. All contributions will be matched 2-to-1 by the Bill & Melinda Gates Foundation

Somalia: Call to Action Meeting for Polio Outbreak Response

What is that Polio Picture? On March 21, the Federal Government of Somalia, the World Health Organization (WHO), and members of the Global Polio Eradication Initiative (GPEI) recommitted to stopping the ongoing outbreak of circulating poliovirus type 2 (cVDPV2) in Somalia at a three-day meeting convened in Nairobi, Kenya. High-level delegates at the meeting included HE Fawziya Abikar Nur, Federal Minister of Health and Human Services, Dr. Mamunur Rahman Malik, WHO Representative to Somalia and Head of Mission, alongside senior representation from the Bill & Melinda Gates Foundation (BMGF), the Centers for Disease Control and Prevention (CDC), Rotary International, Save the Children, the United Nations Children’s Fund (UNICEF), and other UN agencies and partners.

Together, the Government and GPEI partners, which include WHO, UNICEF, the BMGF, CDC, Rotary International, GAVI, the Vaccine Alliance, and other key partners endorsed the Somalia Polio Eradication Action Plan 2022 to reaffirm their commitment.

The Somalia Polio Eradication Action Plan 2022 outlines a four-point call to action to stop the spread of the current outbreak, which is one of the longest-lasting cVDPV2 outbreaks to be reported so far.

SAGE April 2022 – Convening this month in Geneva, the Strategic Advisory Group of Experts on Immunization (SAGE), the global advisory body to the World Health Organization (WHO) on all things immunization, urged concerted action to finish wild polioviruses once and for all.

The group, reviewing the global wild poliovirus epidemiology, highlighted the unique opportunity, given current record low levels of this strain. At the same time, it noted the continuing risks, highlighted in particular by the detection of wild poliovirus in Malawi in February, linked to wild poliovirus originating in Pakistan.

On circulating vaccine-derived poliovirus (cVDPV) outbreaks, SAGE expressed concern at continuing transmission, in particular in Nigeria which now accounts for close to 90% of all global cVDPV type 2 cases, as well as the situation in Ukraine, and its disruption to health services, urging for strengthening of immunization and surveillance across Europe.  It also noted the recent detection of cVDPV type 3 in Israel in children, and in environmental samples in occupied Palestinian territories, urged high-quality vaccination activities, and strengthened surveillance.

Preparing for the post-certification era, the group underscored the importance of global cessation of all live, attenuated oral polio vaccine (OPV) use from routine immunization, planned one year after global certification of wild poliovirus eradication.  To ensure appropriate planning, coordination, and implementation, the group endorsed the establishment of an ‘OPV Cessation Team’, to consist of wider-than-GPEI stakeholder participation and ensure leadership on all aspects of OPV cessation.

Concern Over unfolding crisis in Ukraine

What is that Polio Picture?  The Global Polio Eradication Initiative (GPEI) is extremely concerned about the unfolding effects of the current crisis in Ukraine on the country’s health system.  A functioning health system must be kept neutral and protected from all political or security issues affecting countries, to ensure that people have continued access to critical and essential care.
